SOURCE
These services are funded through Medicaid Home and Community-Based Services (HCBS) Waiver Programs. They are designed to support individuals who meet clinical and financial eligibility requirements for nursing facility level care but prefer to receive services at home or in their community.
ICWP
Supports individuals with physical disabilities who require a nursing facility level of care but choose to remain at home. It provides personal care, assistance with daily living activities, and supportive services that promote independence, safety, and quality of life in a home-based setting.
CCSP
Offers in-home and community-based support for elderly or functionally impaired individuals who qualify for nursing home level care. Services include personal care assistance, homemaker support, and case management to help clients maintain independence and avoid institutional placement.
